Referral Process
Referring a patient is simple and efficient.
Download our physical therapy prescription form and fax it along with the patient’s contact information. Our administrative team will contact the patient within 1-2 business days to schedule their initial evaluation.
Communication & Follow-Up
We believe collaboration leads to better patient outcomes.
After the initial evaluation, you will receive a detailed report outlining the plan of care. Throughout treatment, we provide updates as needed, along with a discharge summary upon completion.
If you would like to discuss a specific patient or treatment plan, our providers are always available to connect directly.
